Purpose:

Participate in Commencement ceremony before completing degree requirements.

Policy:

Requests to participate when student needs more than 15 credits to complete undergraduate degree requirement must be approved by Dean. Automatic permission to participate is granted only to those students who need 15 credits or fewer to complete degree requirements.

Instructions:

1. Complete form.

• Type your name as you want it to appear on the commencement program.
• Include your Student ID number (SID) so we can identify you correctly.
• It is a convention to spell out the middle name and not use an initial.
• Include your degree (BA, BPH, BS, BSA, BSE, BSM) and primary major. These impact how you are listed in the commencement program.
• Include cumulative GPA and major GPA; you may find these GPAs in your degree audit.

2. If you need more than 15 credits to complete graduation requirements, briefly note your plan for completing them (e.g., will finish in fall semester; BSM-MAC walking with BSM cohort; etc.)
3. Sign and date form (see Options for Signing Electronic Forms, below).
4. Email completed form to academic advisor.
